The role you'll contribute:
Care Exemplary Practice o Utilizes the Nursing Process (Assess, Plan, Implement and Evaluate) to provide optimal patient care.
o Demonstrate accurate assessment skills appropriate to my patient population and based on evidence, policy and practice standards when caring for all patients regardless of complexity or need.
o Assure patient safety by adhering to established policies and procedures and utilizing patient safety resources such as mPPID.
Implement a plan of care according to identified physical, psychosocial and spiritual needs.
o Demonstrate the ability to prioritize nursing interventions based on patient needs and in conjunction with unit activities and resources available.
o Actively participate in bedside shift report, safety huddles, multidisciplinary patient rounds and hourly rounding to gather all information necessary to care for my patients, anticipate their needs and provide for a smooth hand-off to other members of the health care team.
o Provide patient teaching/discharge planning to promote optimal patient outcomes.
o Provide routine and specialized teaching/discharge planning throughout the patient stay to prepare the patient/family/significant other regain independence prior to discharge.
o Provide individualized teaching plan based on assessed learning needs, education level, language spoken and level of understanding.
o Provide patient and family/significant other with a completed Teaching/Discharge Plan which includes specific information regarding medications, follow-up and community resources as needed.
o Document all pertinent information (health history, assessment, test results, plan of care, teaching and patient response) in a timely manner according to departmental policy.
Shared Leadership Transformational Leadership o Accept accountability for problem-solving issues and work with leaders to resolve clinical issues.
o Participate in shared decision making by working on committees and/or providing input to committee representatives.
o Participate in performance improvement activities and make recommendations for revisions to practice based on evidence and data collection.
Interprofessional Relationships o Seek assistance from others and act as a resource to coworkers in order to provide optimal patient care through teamwork.
o Coordinate with other resources (e.

case management, home health) as needed.
o Contribute to the growth of others by providing feedback through peer coaching.
o communicate to other members of the health care team and serve as a patient advocate when necessary.
Nursing Advancement New Knowledge o Attend all hospital, unit mandatory and continuing education requirements.

EDUCATION AND EXPERIENCE REQUIRED Associate's Degree in Nursing No Previous Experience EDUCATION AND EXPERIENCE PREFERRED Bachelor's Degree in Nursing or obtained within 5 years of employment LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Kansas Registered Nurse License BCLS obtained through the American Heart Association LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
Additional certifications/registrations are required or preferred by specific clinical area of practice.
RN must demonstrate certification/registration upon hire or agree to obtain requirements(s) within 1 year of hire unless deemed otherwise by manager.
Please note that BCLS and ACLS must be courses provided by the American Heart Association.
